Homecare services after discharge Before discharging you home, your treating doctor will make necessary prescriptions for homecare service for you if appropriate. This can be continuous medication monitoring, wound care, continuous nutritional regime, physiotherapy and rehabilitation. Our homecare team will visit and discuss the plan and the cost with you. Upon your acceptance and payment, services will be then provided which will help you to have faster, better recovery. Post Discharge follow-up call Within 1–2 days after discharge, you may receive a follow-up call from our nurse who checks on your recovery and reminds you of any scheduled follow-up appointments.
