FV Hospital, its employees, and its physicians who have accepted the appointment via the hospital website do not provide any medical services, diagnosis, or treatment procedures to the person making the appointment request until said person has registered in person at FV Hospital. Regarding any dispute or claim (including claims for personal injury) relating to healthcare services I received at FV Hospital that cannot be resolved through mutual agreement, jurisdiction shall belong to a competent third party in accordance with the laws of Vietnam. FV Hospital shall adhere to its Personal Data Protection Policy, and I hereby: Commit that all information provided in this form is and will remain accurate and complete until I/my dependent(s) notify FV Hospital of any changes.
